Other International Travel Forms
Travelers must submit a Single PDF Document to the Office of International Affairs that includes (if a component is not applicable to you, please note that in the packet):
- Justification Letter from traveler addressed to the Provost outlining the request for travel including connection to Preeminence 2030, travel dates, conference dates, and funding source. Letter must be written on official letter head.
- Faculty Notice of Absence Form or Notice of Leave Form (should include travel dates and conference dates in "Reason for Absence")
- International Travel Registry Form (please be sure to circle/highlight status line)
- Presentation or Research Invitation/Acceptance Letter (in English)
- Conference or meeting overview page
- Meeting/Conference-at-a-glance agenda
- Registration page including the cost of attendance
- Sample Flight and Lodging cost information
- OANDA Currency Converted (full costs) if estimates are in foreign currency
- Insurance Roster (Receipt of payment due after the Office of the Provost approves travel)
- Copy of passport and/or required U.S. re-entry documents
- Copy of required visa for entry to foreign country (if necessary)
- Safety Assessment (search for destination, review travel advisory level, take a screenshot of the advisory page and add it to your packet) - Level 3 Do Not Travel

The Single PDF Document with all required documents (noted above) should be uploaded to Office of International Affairs' Travel Registry.
