Graduate Students Health Requirements

Graduate Students are not required to have a physical examination on file. You cannot register, however, until you have completed and submitted the Medical History / Immunization requirements via On-Line Student Portal. 

North Carolina state law (General Statute 130A-155.1) requires anyone entering college to present a certificate of immunization that documents their compliance with all required immunizations.

Students registered only in off-campus courses:

Distance Learners: The NC Immunization Law does not mandate you to complete any forms. Please contact the Medical Records department about your intent to enroll. Please note that at any time you are considered an on-campus student, you will have to meet NC Immunization requirements.

Students only attending classes scheduled at 5:00 pm or later:

Post-Baccalaureate Studies (PBS): Students who take 5 or more hours on campus, before 5:00 p.m. are required to meet the NC Immunization requirements. Please download the Medical Health Packet form.  See the online student portal.

Students enrolled only in weekend classes  

Students taking a course load of four (4) credit hours or less per semester:

Teacher Certification / Certificate: Students who are considered visitors taking 3 hours or less, attending the University to complete certificate courses, must individually contact the Medical Records office for additional advisement.
