Documentation Guidelines

The student self-identifies with the Office of Accessibility Resources and submits the Intake Forms along with current documentation to the Director to be reviewed. Please refer to the Documentation Guidelines

  • Once the paperwork is received and reviewed, the student will be notified.
  • If the documentation substantiates the need for accommodations, the student will be given an opportunity to meet with a staff member to discuss reasonable accommodations and other processes associated with the office.

Please note: Registering with the Office of Accessibility Resources (OARS) can take up to 2 weeks to complete. Please submit documentation as early as possible.

Disability documentation is confidential information from an appropriately certified professional who is knowledgeable about you and your condition. Such professionals include physicians, educational psychologists, therapists, mobility specialists, and rehabilitation counselors. Documentation is used to determine eligibility for disability services and accommodations.


Once documentation is reviewed by the Director, students are contacted via student email address to set up an intake appointment. 

During the Intake meeting, the Access Coordinator will meet to review various services offered through our office as well as services available campus-wide. We also work together to review and determine accommodations substantiated by the documentation. If you are a distance education student, we are able to schedule these appointments over the phone.


Students who receive accommodations for courses at N.C. A&T State University is required to complete the Accommodation Request Form each semester. Once complete, a letter in PDF format will be scanned and emailed to the student and the instructor of each course electronically. The letter explains all approved accommodations. It is the responsibility of each student to communicate with the instructor upon receipt.

 A&T State University is committed to supporting students with disabilities by allowing emotional support animals (ESAs) when an ESA is necessary for them to be able to use and enjoy their residential living space. The Emotional Support Animal (ESA) Policy applies to all students and explains the specific steps necessary to request an emotional support animal.


A disability-related absence is when a student is absent due to the direct effects of a documented disability. This accommodation can be implemented differently in each course as the essential elements and design of a course have to be taken into consideration. If verification of absences is requested by faculty to excuse an absence, the following form will have to be completed by a licensed professional. 


This manual is intended to be a reference for current students, faculty, staff, prospective students,
and parents of students about the processes and procedures of the Office of Accessibility Resources at N.C. A&T State University. Information is presented in general; all accommodation requests are handled on a case-by-case basis.
