SELF-STUDY QUESTIONNAIRE

 Dear SOT Alumni:

      It is delightful to communicate with you and to now officially address you as an alumnus of the School of Technology.  I hope that the past few months have continued to be filled with the anticipation of an exciting future, both personally and professionally.  We also want you to know that we still consider you a member of the Aggie Family and encourage you to keep us informed of life changes. 

     The School of Technology continually strives to improve its programs to better serve students who will follow in your foot steps.  To facilitate this process, I am conducting a survey of most recent graduates; specifically those who graduated in December 2005 and May 2006.  Since your degree was conferred during one of these dates, I am requesting your assistance.  You may assist me by completing the enclosed survey.  To complete the on-line survey, you must enter the assigned CODE #.  Your CODE #, located at the top right corner of your mailed questionnaire, will enable you to access the on-line survey.  If you have not received a mailed questionnaire, but desire to complete the on-line survey, please contact me at davisrj@ncat.edu or call (336) 334-7359.  

     Additionally, feedback from your immediate supervisor is also being requested.  Supervisors may access the survey at http://www.ncat.edu/~sot/survey.htm. Further, in completing the on-line form, please ask your supervisor to use the same Code provided to you.  To ensure anonymity, no names will be used in reporting survey results.  Therefore, please provide your candid response.   If you know other SOT alumni (i.e., December 2005 and/or May 2006), please encourage them to complete the survey.  Your time and cooperation are greatly appreciated.  Again, best wishes.
