Instructions on How to Use and Fill out Forms
1. The local computer you are using needs to have Adobe Reader (Adobe LiveCycle Reader Extensions installed), Adobe Acrobat Standard, or Adobe Acrobat Professional installed.
2. Click the File menu and the command "Save the Page As" and direct it to the desktop of the local computer. Save the file under another name, for instance, use the last name to save the form.
3. Then, from the desktop copy fill in the fields and save the file after changes to the form or when the form has been completely filled out. Save the file frequently instead of at the end of filling out the form.
4. Click Save.
5. From this point the form retains the typed information and can be printed or emailed.
6. Do not use the web browser to fill out the form. |
