All
international
students must
provide documented
proof of required
immunizations.
Students who fail
to comply with
this requirement
will not be
allowed to
register for
classes. The
Health Center will
place a
‘registration
hold’ on your
student account.
The hold will not
be lifted until
all medical
records are turned
in and
immunizations are
completed.
The Health Center
requires all
international
students to have a
Tuberculin Skin
Test before
receiving health
clearance. All
undergraduate
students are
required to have a
physical
examination (refer
to Medical Form).
Graduate students
are not required
to have a
physical.

website. You will
also be able to
download the
medical form (must
be Adobe Acrobat
[.pdf]). The form
should be
completed as
instructed and
returned to the
Health Center.
