Procedures for Athletics Physicals
All student athletes are required to have a completed entrance physical evaluation/medical history form, signed by a physician, on file in the Student Health Center. If the student athlete does not have a medical form on file, he may choose to do one of the following:
1. Submit the completed form at the time of the athletic physical;
2. Return the completed form personally or by mail to the Student Health Center prior to the scheduled athletic examination; or,
3. Take the form to a local physician for completion or notify the athletic trainer for the need of an outside physical.
All athletic physical examinations will be given in the Student Health Center by prearranged schedule only!!
During the organization of athletic group physicals, i.e., football, basketball, baseball, etc., the following guidelines should be adhered to:
1. The athletic trainer should request all coaches to submit a list which includes the name, social security number and classification of all freshmen, veteran, transferring, and walk-on athletes at least three weeks prior to the scheduled examination.
2. At least two weeks prior to the scheduled examinations, the trainer should submit this list to the Student Health Center in order that medical records of each candidate may be retrieved and reviewed. The Center will then inform the trainer of any candidate not having a medical record on file. The trainer will immediately inform the coach, who will send the candidate directly to the trainer so that he/she may obtain the proper medical forms.
3. After the scheduled date for the physical, all walk-ons must be sent to the head trainer, by the coach, and the head trainer will make an appointment with the Health Center as outlined above.
4. The team physician and the campus physician will be the final authorities regarding defects and/or limitations that will exclude an athlete from athletic competition.
5. Any athlete with a pre-existing injury or condition requiring consultation from a specialist must assume the cost and/or file appropriate insurance information for the service.
6. An athlete needs only one physical exam for each sport.
7. Any injury incurred through athletic participation should be reported to the athletic trainer and University physician immediately. If additional consultation is needed and/or requested, the Health Center can make the proper referral or recommendations.
8. When an athlete is not involved in an athletic practice or contest and needs medical attention, he/she should contact (a) the Student Health Center, (b) the team trainer or coach, or (c) if the above are unavailable, the local hospital emergency room and report to the Health Center as soon as possible.
9. All outside referrals and consultations should be made through the University physician whenever possible. The head trainer will be informed continuously about pertinent injuries and/or illnesses and follow-up evaluations on all athletes.
