North Carolina Immunization Requirements and Exemptions

North Carolina state law   (General Statute 130A-155.1) requires all undergraduate and graduate students taking 4 hours or more on campus to provide documented proof of the required immunizations before registering for classes.

This Report of Medical History is designed to collect information about your health history and to verify that your immunizations are in compliance with the NC Immunzation Law.

The statute applies to all college and university students, except:
¨ Students registered only in off-campus courses;
¨ Students only attending classes scheduled 5:00pm or later;
¨ Students enrolled only in weekend classes;
¨ Students taking a course load of four (4) credit hours or less per semester

If at any time, the above student status changes to: on-campus courses, course load of more than four credit hours, on-campus residence, and/or enrollment in day-time courses, a certificate or record of immunization must be presented on or before the date the person first registers for a quarter or semester.

REQUIRED Immunizations

√ DTP (Diptheria, Tetanus, Pertussis) or Td (Tetanus, Diptheria) or Tdap (Tetanus, Diptheria, Acellular, Pertussis)
√ MMR (Measles, Mumps, Rubella)
√ Polio or poliomyelitis
√ PPD or TB skin test (based on foreign travel)

√Hepatitis B
Vaccine Requirements Based on Student Age (vaccine / number of doses required)

Students 17 years of age and younger:

3 DTP (Diphtheria-Tetanus-Pertussis) or TD (Tetanus-Diphtheria) doses.

1 Td/Tdap (Tetanus-Diphtheria and/or Tetanus-Diphtheria-Pertussis) dose must be within the last 10 years.

3 Polio doses.

2* MEASLES (Rubeola) one dose on or after age 12 months and before age 16 months and a second dose before enrolling in school for the first time. (2MMR doses meet this requirement.)

1** RUBELLA (German measles) dose. (after first birthday).

2** MUMPS doses (both after first birthday)

3 Hepatitis B doses.

 

Students Born In 1957 Or Later And 18 Years Or Age Or Older:

3 DTP (Diphtheria-Tetanus-Pertussis) or TD (Tetanus-Diphtheria) doses.

1 Td /Tdap (Tetanus-Diphtheria and/or Tetanus-Diphtheria-Pertussis) dose must be within the last 10 years.

2* MEASLES (Rubeola) one dose on or after 12 months of age, the 2nd before age 16 months of age and second dose before enrolling in school for the first time. (2 MMR doses meet this requirement.)

1** RUBELLA (German measles) dose. (after first birthday.)

2** MUMPS doses. (both after first birthday.)

 

Students 50 Years Of Age And Older:

3 DTP (Diphtheria-Tetanus-Pertussis) or TD (Tetanus-Diphtheria) doses.

1 Td/Tdap (Tetanus-Diphtheria and/or Tetanus-Diphtheria-Pertussis) dose must be within the last 10 years.

 

International Students ONLY

Students are required to have a negative Tuberculin Skin Test (TST) (less than 5mm) within the 12 months preceding the first day of classes. Documentation of a TST is acceptable only from a United States facility trained in public health or occupational medicine or equivalent US Institution. If the TST is equal to or greater than 5mm, you will be required to schedule a TB evaluation with a Campus Health provider before your immunization status can be cleared. A chest x-ray may be required. Only chest x-rays performed within the United States will be accepted.
