Immunization Information for All Incoming Students
North Carolina state law (General Statute 130A-155.1) requires all undergraduate and graduate students taking 4 hours or more on campus to provide documented proof of the required immunizations before registering for classes (see specific immunization requirements and exemptions to the law below).
This Report of Medical History is designed to collect information about your health history and to verify that your immunizations are in compliance with the NC Immunization Law.
- Mail forms by May 1st for students entering in the Fall Semester
- Mail forms by December 1st for students entering in the Spring Semester/ Summer Session

Immunization information may be submitted to the NCA&TSU Student Health Center in one of four ways:
1. Print a copy of the Medical History Packet and submit to a physician to complete and sign;
2. If you have a copy of your immunization records that is stamped or signed by the administering physician/health agency and includes dates on which immunizations were administered, you can simply submit a copy of these records. Please include your name, student identification number (Banner 950...Id number) and date of birth on all pages submitted;
3. If immunization information is included on high school or other transcripts, request from that school that a copy of the transcripts be sent to the NCA&TSU Student Health Center.
NOTE:
Because transcripts contain academic information, a copy of your transcripts is not sent to the health center by the NC A&T SU Admissions Office;
4. You may bring your records to the Student Health Center for immediate review.
Transfers:
If you have attended another college or university that also required immunizations, contact the former university's health center to request that a copy of your records (if still on file) be mailed to the NC A&T SU Student Health Center.

Hepatitis B: protects against this serious liver disease, which may result in liver cancer. Hepatitis B is spread by contact with blood and bodily fluids.
Meningococcal "Menactra/Menomune": protects against the most common strains of Meningococcal Meningitis. Recommended for first-year students living in the residence halls.
Influenza/flu: protects against this acute, contagious respiratory infection. Offered seasonally.
