Student Complaint Form Instructions

STEP-BY-STEP
HOW TO FILL OUT STUDENT COMPLAINT FORM
OFFICE OF VICE CHANCELLOR FOR STUDENT AFFAIRS
 

Step 1: Enter your First and Last Name in the indicated box

Step 2: Enter your 9-digit N.C. A&T Banner ID number

Step 3: Enter your Classification (Freshman, Sophomore, Junior, Senior, Graduate, Doctoral)

Step 4: Enter the College/ School your Major is under (College of Agriculture and Environmental Sciences, College of Arts and Humanities and Social Sciences, College of Business and Economics, College of Education, College of Engineering, College of Health and Human Sciences, College of Science and Technology, Joint School of Nanoscience and Nanoengineering, The Graduate College)

Step 5: Enter your Major

Step 6: Enter your telephone number

Step 7: Enter your N.C. A&T student email address

Step 8: Click the “Next page” button

Step 9: Click the button “Yes” or “No” to indicate if complaint is a grade appeal

Step 10: If the complaint is a grade appeal click on box and type the reason for the appeal

Step 11: Click the button “Yes” or “No” to indicate if complaint is against a person or department

Step 12: If the complaint is against a person type the faculty/ staff full name

Step 13: If the complaint is against a department type the department name

Step 14: Click on box and write a brief description of complaint or upload a document (Click on “Browse” button to attach a file)

Step 15: Click the button “Yes” or “No” to indicate if you spoke with an N.C. A&T faculty or staff member regarding the complaint

Step 16: Click on box to type attempts to resolve the matter

Step 17: If you have additional documents pertaining to complaint, click the “Browse” button to attach file

Step 18: Type full name in “Student Signature” box

Step 19: Review all information on page 1 and 2

Step 20: Click “Submit” button

Step 21: Confirmation screen will appear
