Graduate Student Health Care Requirements

The Student Health Center (SHC) welcomes you to the University. A variety of medical and health services are provided for students enrolled in campus courses who pay the health fee. Services are offered to registered students between the hours of 8:00 a.m. and 7:00 p.m. (Monday-Friday).

The Graduate School provides a variety of programs for educational enhancement. Below are the current immunization requirements for all students prior to registration. You may find on the web-site the fee charges for immunizations administered at the Student Health Center for those students who arrive on campus and do not have copies of their vaccination records. Graduate Students are not required to have a physical examination on file. You cannot register until you have completed and submitted the Medical History / Immunization requirements via On-Line Student Portal. 

North Carolina state law (General Statute 130A-155.1) requires anyone entering college to present a certificate of immunization that documents their compliance with all required immunizations.

The statute applies to all college and university students, except:

  • Students registered only in off-campus courses;
  • Students only attending classes scheduled 5:00 pm or later;
  • Students enrolled only in weekend classes;
  • Students taking a course load of four (4) credit hours or less per semester

The following instructions will assist you in your transition to the University and completing NC Immunization requirements:

Distance Learners: The NC Immunization Law does not mandate you to complete any forms. Please contact the Medical Records department about your intent to enroll. Please note that at any time you are considered an on campus student, you will have to meet NC Immunization requirements.

Post Baccalaureate Studies (PBS): Students who take 5 or more hours on campus, before 5:00 p.m. are required to meet the NC Immunization requirements. Please download the Medical Health Packet form located at the above web-site address.

Teacher Certification / Certificate: Students who are considered visitors taking 3 hours or less, attending the University to complete certificate courses, must individually contact the Medical Records office for additional advisement.

Students Born In 1957 or Later: 

3 DTP (Diphtheria-Tetanus-Pertussis) or TD (Tetanus-Diphtheria) doses.

1 Td /Tdap (Tetanus-Diphtheria and/or Tetanus-Diphtheria-Pertussis) dose must be within the last 10 years.

2* MEASLES (Rubeola) one dose on or after 12 months of age, the 2nd before age 16 months of age and second dose before enrolling in school for the first time. (2 MMR doses meet this requirement.)

1** RUBELLA (German measles) dose. (after first birthday.)

2** MUMPS doses. (both after first birthday.)

 Students 50 Years Of Age And Older

3 DTP (Diphtheria-Tetanus-Pertussis) or TD (Tetanus-Diphtheria) doses.

1 Td/Tdap (Tetanus-Diphtheria and/or Tetanus-Diphtheria-Pertussis) dose must be within the last 10 years.

First Time Students please visit

International students are required to provide proof of a negative PPD/Tuberculin skin test within 12 months proceeding the first day of classes.  If your PPD/TB is positive a chest x-ray may be performed within the United States.
