Cadets or enlisted
soldiers who contract and become SMP participants must
complete selected Cadet Command Forms/contracts as well as
USAR/ARNG forms. In addition to the forms necessary for
contracting, the following forms are necessary for SMP
participation:
DA Form 4824-R
(SMP agreement between Cadet Command and USAR unit) or NGB
Form 594-1 (SMP agreement between Cadet Command and ARNG
unit).
Copy of DD Form
214.
SMP Letter of
Recommendation from National Guard or Reserve Unit Commander.
Verification of
US Citizenship.
Past medical
exam (SF 88 and SF 93).
Obtain letter
of acceptance from the Professor of Military Science.
To ensure
service in the ARNG/USAR upon commissioning, the SMP cadet
must obtain a guaranteed reserve forces duty (GRFD) control #
from Cadet Command and complete Cadet Command Form 202-R
(Guaranteed Reserve Forces Duty Non-Scholarship Endorsement)
or Cadet Command Form 203-R (Guaranteed Reserve Forces Duty
Scholarship Endorsement).
